Required Documents
- Birth certificate for minors;
- Taxpayer Card;
- A color passport-sized photo;
- Proof of professional activity/income source;
- Product and service subscription form (to be filled out at the Branch);
- Individual Information Form for each participant (to be filled out at the Branch);
- Proof of Residence, utility bill, rental contract;
- Identification Document (nationals): Identity Card;
- Identification Document (foreigners): Resident Card or Passport with valid visa;
Proof of address (if the presented identification document does not mention the address or if the mentioned address is different from the current residence).
